How to Create and Customize Addo Forms
Addo Forms help you collect data, signatures, and responses in a streamlined process.
Use them for contracts, intake forms, or questionnaires to provide a professional experience.
Steps to Create a Form
Requirements: Admin or group-admin rights.
Go to 'Settings' > 'Forms'.
Click 'Create new form':
Enter the required details:
Template name (internal use).
Document filename (optional).
Form title (visible to recipients).
Description (optional).
Select a signing template (orange box on screen):
Signing Templates
Addo Forms require a signature template, which controls the signing flow and any additional documents to be signed.
Organizing Your Form with Sections
Forms are divided into sections to group content logically:
You can keep all questions in one section or add multiple sections (boxes).
Rearrange sections by dragging the two horizontal lines in the top-left corner:
What recipients see:
Form title and description.
Questions grouped by sections.
Adding Questions (sender options)
To add questions or answers, click 'Add' within a section. You can choose from various field types and framing options like title and text.
Basic fields:
Title: Labels the section; editable only within the form.
Text: Adds explanations or context; editable within the form.
Single-line text: For short answers (e.g., name, age).
Multi-line text: For longer responses (e.g., address, statements).
Choice fields:
Checkboxes: Allow multiple selections.
Radio buttons: Allow one selection and can trigger dependent questions:
To create dependencies with radio buttons:
Enable Has dependents.
Click Add dependent component under options:
Select the dependent question type (e.g., multi-line text).
The dependent question appears based on the chosen answer.
Other fields:
File upload: Recipients can upload documents or images.
Image: Insert an image into the form.
Date: Select a date using a calendar picker.
Mandatory and Prefilled Fields
Mark questions as mandatory to ensure recipients cannot submit without completing them.
Use prefill when sending to enter answers yourself before sending (e.g., salary details in a contract).
Enable public link
Forms can be sent directly or as a public link (ideal for enrollment, claims, etc.). See our article on public links for more details.
Finish and Save
When your sections and questions are ready, review the form details. Click 'Save' to finalize your new Addo Form.
It will now be available for sending or sharing via a public link.
Recipient View (answers)
Recipients will see the form fields and signing elements as configured:
Titles, text fields, single-line, and multi-line answers.
Checkboxes, radio buttons, file upload, images, and date fields.
Signing template fields as defined in your form.
Note: Coloring and company branding are set in Design settings and can only be changed by an administrator
