Use Your Own PDF as a Template in Addo Forms

This article describes how to use your own PDF as a template in Addo Forms by creating editable fields, mapping them, and customizing output

With Addo Forms, you can decide exactly how the final document will look after it has been filled out. This allows you to design the output according to your company’s fonts, colors, and visual identity.

By uploading your own PDF and mapping Addo Form fields directly into it, all submitted data will be merged into your PDF instead of the default Addo layout. The customized PDF is then presented for signing once the form has been completed.


What Does It Mean to Use Your Own PDF?

Using your own PDF as a template means you control the design of the final signed document. Without a custom PDF, Addo Forms generates a document in the default Addo Sign layout.

Process without custom PDF

  1. The user fills in the form.

  2. Clicks Save at the bottom.

  3. A PDF is automatically generated based on Addo’s standard formatting.

Process with custom PDF

  • You design the layout (or we can help you).

  • Data from Addo Forms is merged into your PDF, preserving your fonts, colors, and formatting.


Creating Editable Fields in a PDF

Editable fields are placeholders in a PDF where information can be entered (e.g., text fields, checkboxes, or radio buttons).

👉 To create editable fields, you need Adobe Acrobat Pro (paid version) or another advanced PDF tool.

Steps in Adobe Acrobat:

  1. Open your PDF in Adobe.

  2. Search for and select Prepare Form.

  3. You can now insert editable fields.

Common field types for Addo Forms mapping:

  • Text field – Works with text, date, or larger text areas in Addo Forms.

  • Checkbox – Matches checkbox elements in Addo Forms.

  • Radio button – Matches radio button fields in Addo Forms.

Example with "Name":

  1. Select Text Field from the toolbar.

2. Place it in the correct position in the PDF.

3. Name the field logically (e.g., “FirstName” or as here, “Landlord) so it matches the intended Addo Form field.

When finished, click Close and save the document.


Mapping Addo Form Fields to Your PDF

Once your PDF has editable fields, you can upload it to Addo Forms and map the fields.

Steps:

  1. Go to Settings in Addo Sign.

  2. Select Forms.

  3. Choose the Addo Form you want to apply the PDF template to.

  4. Upload your PDF with editable fields:

    • Enable Show data fields to display mapping options.

    • Tick PDF schema.

    • Click Upload PDF schema and select your file.

Mapping fields:

  • Once uploaded, Addo will display all Data Output IDs from your Addo Form.

  • Match each Addo field with the corresponding PDF field.

This is an example of how a PDF form has been set up. This shows what it will look like for the customer when they fill it out.

💡 Tip: Start typing the field name in the mapping box and choose from the dropdown list. This avoids issues with uppercase/lowercase differences, as Addo is case-sensitive.

Finalizing:

  1. Map all fields correctly.

  2. Scroll to the bottom.

  3. Click Save.


✅ Result: Your Addo Form now uses your own PDF template. When users complete the form, their input is merged directly into your PDF design, which is then presented for signing.
